Appointments are scheduled from 8:30 am - 3:30 pm.

The following guidelines MUST be followed in order for us to complete fingerprinting:

• Please complete the online ADE Background Consent Form.

• Please complete the payment for the Background Check.

• Print both forms to bring to your appointment.

• All online consent forms and payment forms must be complete before coming to the CoOp for fingerprinting.

If the Arkansas Notary is available we can notarize the Child Maltreatment DHS form. You are responsible for uploading the CMCR form to AR DHS. You can access this form here.
